La imagen diagnóstica pasa de ver la forma de los tejidos a analizar su comportamiento

"Los equipos de resonancia ofrecen cada día mayor resolución. La diferencia en los últimos años radica en que se puede analizar el comportamiento bioquímico y metabólico de las lesiones e, incluso, identificar in vivo información que antes nos era imposible porque solo teníamos datos morfológicos", explica Mar Jiménez, jefe asociado de diagnóstico por la imagen de Hospital Universitario Quirón Madrid. En la actualidad, el clínico tiene la opción de elegir entre diferentes tipos de visualización de la lesión según lo que esté buscando: alteraciones anatómicas, sangrados, modificaciones grasas, lesiones de pequeño tamaño. La novedad estos últimos años se produce cuando ya se ven las lesiones tumorales y, además, se dispone de datos sobre lo que está pasando dentro de ellas. "El ser humano es, en un porcentaje muy alto, agua. El movimiento de las moléculas de agua en cualquiera de nosotros es aleatorio; sin embargo, cuando aparecen lesiones, este movimiento se altera y estas variaciones son lo que muestran las imágenes de la resonancia magnética. La reducción del movimiento de las moléculas de agua nos indica no solo que existe una lesión, sino que también nos informa sobre su comportamiento biológico". ESTUDIOS DE DIFUSIÓN Este tipo de análisis de la información procedente de la resonancia magnética -que se denomina estudios de difusión- se está utilizando para ver cómo son celularmente y cómo se comportan las lesiones de muchas clases. "Se empezó a emplear para el diagnóstico de los infartos cerebrales y, posteriormente, se extendió al cáncer de mama y a los tumores ginecológicos, de próstata, de recto y óseos. Gracias a los estudios de difusión se aprecia la estructura del tumor y se nos orienta sobre su posible agresividad", explica Javier Carrascoso, jefe asociado de diagnóstico por la imagen de Hospital Universitario Quirón Madrid. "Los estudios de difusión son muy útiles para valorar si el tratamiento está funcionando o no sin necesidad de realizar pruebas diagnósticas más agresivas. Podemos observar, mediante estudios sucesivos, si el tumor presenta mayor necrosis o está menos vascularizado, lo que quiere decir que el tratamiento de quimioterapia o radioterapia está siendo efectivo". Las imágenes de resonancia magnética permiten medir si un tumor tiene más o menos densidad celular y la cantidad de espacio que existe entre unas células tumorales y otras. "Estos datos dan pistas sobre las características de la lesión neoplásica y ayudan a orientar el diagnóstico", añade. Con el tiempo, según se vaya comprobando que la información que ofrece la resonancia magnética se correlaciona con la evolución clínica de los pacientes, se podrán evitar procedimientos más agresivos, como las biopsias sucesivas.
